This is a reminder that we need an updated copy of your player’s latest physical form. We are required by Massachusetts Law and by the Board of Health of the Town of Wellesley to have a copy of the form for camp. The health form may not be more than two years old at the start of camp. (forms dated before August 11, 2012) need to be updated. We have a list of past forms and will try and get it up on the web site for reference soon. All new players need to provide a copy of an updated health form. Camp begins Monday August 11, 2014 and players will not be allowed to participate in camp if the form is not received.
These requirements are needed immediately. I am meeting again with the Wellesley Board of Health and need to show progress. Forms can be scanned to a PDF File and emailed along with any questions you may have to email@example.com or faxed to (617) 789-4112.
